Case management

We will connect you with a Case Manager who will take the time to get to know you well, and identify your needs and aspirations. With you, they will create a personalised care plan that will support you to remain happy, healthy and safe in your home, as well as:

  • keeping you connected to family and friends
  • assisting you in navigating the complex aged care system
  • helping you plan for the future.

Your highly qualified, dedicated Case Manager can support you in setting up and accessing services within your budget including, but not limited to the following:
